Piwik 3.0 upgrade - LDAP settings blank

Updated to 3.0 - after sorting some permissions, all seems well. But the LDAP plugin seems not happy - I go to Settings > LDAP and it is blank (just see 2 narrow, empty, white boxes). I first upgrated, then removed and reinstalled, the LDAP plugin, with no change.

Any ideas as to how best to check what’s wrong?

Are you using the Ldap 4.0.0 ? if so please create a ticket in the bug tracker directly: GitHub - matomo-org/plugin-LoginLdap: LDAP authentication and synchronization for Piwik.
