Delete button unresponsive

Piwik 2.9.1:
In Settings->Websites, when clicking on Delete (website), nothing happens on any of the four websites in my list. Tried IE11, Firefox, Chrome.

I have the same problem after updating to 2.9.1
I can not delete users or websites.
I Have done a fresh new install with a new database and the install falls into a “Database Upgrade” Loop.
There are no warnings or errors in the apache/php logs.

does it fail on all browsers?
it works for me on few PIwik servers, if you have an issue with 2.9.1 please create an issue in the tracker: Issues · matomo-org/piwik · GitHub

Tried Firefox, Chrome, Safari and IE11 in both standard and compatibility view. Same result. Link get underlined but nothing happens when clicked.
