Writing a plugin with a custom form

Hello there, I’m wondering about writing a plugin with a form inside a controller.
In that page the user must select parameters will be passed in the next GET requests in addition to date, period, idSite core parameters.
Then I would use that custom GET parameters for my plugin needs.

Any suggestion? Is there any example made by existing plugins inside the marketplace?
