Trouble adding a new site

Hello!

I have been using Piwik for a couple years now and it has worked perfectly. It still does, but after the new update (2.14.2) I have trouble adding a new site.

Please see attached image where I am having trouble:

I manually updated Piwik with newest version, but it is still the same.

Anyone have any tips to solve this? :slight_smile:
