Goal tracking based on History Change tracking issue

We have a VueJS app and want to track signups.

When people land on the /setup page we count them as a trial start.

We have 100 - 200 trial starts a day but only count 8 or so in Matomo.

I have history change and pageview change tracking enabled. And app usage is tracked normally.

What could be wrong?

How do you track exactly? can you provide some code examples?
Do you see any error in the JavaScript console?
