Error: Action 'index' not found in the module 'Actions'

I just installed this for the first time. All installation went fine. I have 2 visits on my site. All pages under the Actions tab show this error: Error: Action ‘index’ not found in the module ‘Actions’

How to fix?

Are you using Piwik 2.15.0? which URL do you get this error on?
